General Information : How Do I Add Dependents

Students must select or waive coverage through their online "Student Center" by the published deadline of their first term of enrollment every year. Dependent coverage (for a legal spouse, same-sex or opposite-sex domestic partner, and/or child[ren]) should also be selected at that time.
Note: Students can enroll dependents into the Plan in the middle of the academic year only if they experience a qualifying event (marriage, birth, involuntary loss of coverage from another plan, etc.). Students who have experienced a qualifying event, will need to complete the "Coverage Status Change Form" and submit it along with the required supporting documentation to Gallagher Koster Insurance within 31 days of the qualifying event. A "Dependent Information Form" should also be submitted at that time.
To add dependents through the online "Student Center":
- Go to http://www.buckeyelink.osu.edu and navigate to your "Student Center".
- Scroll down to the "Finances" section.
- Under the "Student Health Insurance" heading, click on "Select / Waive Coverage".
- Choose the appropriate "insurance option" and "academic terms", then click "Next".
- Choose the appropriate "insurance level", fill in the "dependent information", then click "Next".
- "Confirm" your selection, then print or write down your confirmation number.
- Verify your enrollment by viewing your online Account Statement. Your selection should be reflected on your Account Statement within 1-2 business days.
If you are registering a same-sex/opposite-sex domestic partner, you must submit a notarized "Affidavit of Domestic Partnership" to the Student Health Insurance Program Office.
If you would like to be able to fill prescriptions for your dependents at the Pharmacy in the Wilce Student Health Center, please submit the "Pharmacy Access Enrollment Form for Covered Dependents" to the Patient Relations department at the Wilce Student Health Center.
