$ Tier 1: Lowest Cost (On-Campus)
- No copays, deductible, or coinsurance for most services provided by Student Health Services at the Wilce Student Health Center
- Copay and coinsurance amounts for dental and prescription drugs.
$$ Tier 2: In-Network / Select Providers (Off Campus)
- $20 copay for primary and specialty care office visits
- $25 copay for urgent care office visits
- 10% coinsurance for most services
- $0 coinsurance for covered preventive services
- $200 deductible
- $4,000 out-of-pocket maximum
$$$ Tier 3: In-Network / Preferred Providers (Off Campus)
- 40% coinsurance for most services
- $0 coinsurance for covered preventive services
- $600 deductible
- $7,000 out-of-pocket maximum

- The Student Health Benefit plan uses a PPO network
- Inside Franklin County, students use the Ohio State Health Plan Network
- Outside Franklin County, students use the UHC Choice Plus Network which works nationwide

- The Student Health Benefit plan uses a PPO network
- Inside Franklin County, UHC Choice Plus Network but not Ohio State Health Plan Network
