Member ID Cards

Standard policy year coverage begins August 13, 2024. Card availability is contingent upon a confirmed selection in My Buckeye Link (selection + paid fee) and you must allow 1-3 business days for processing.

Policy partners are UnitedHealthcare StudentResources (UHCSR) for medical, mental health, prescription, vision, and pediatric dental primary (members under 19).  Delta Dental of Ohio is the policy partner for adult dental and pediatric dental secondary. 

UHCSR Card (To show when using medical, mental health, prescription, vision, and pediatric dental primary):

1. Go to uhcsr.com/osu or download the UHCSR Mobile App
2. Click “Login to My Account" in upper right corner
3. New users should click “Register Now”
4. Enter your name, date of birth, and Ohio State student ID number.  Create a password you won't forget. 
5. Existing users should click "Sign In"
6. Once inside your account, click “ID Card information” and then click "View or Print ID Card". 

Delta Dental Card (To show when using adult dental and pediatric dental secondary):

1. Go to the Delta Dental website
2. Click "Sign Up" under Member Portal.
3. Enter your name, date of birth, and Ohio State student ID number.

Members using assistive technologies that experience accessibility issues may contact OSU Student Health Insurance at 614-688-7979 Monday through Friday during regular business hours for assistance in obtaining a temporary and permanent Member ID card. 

Permanent Cards: 

UHCSR does not mail you a permanent ID card.  See the instructions above to access and print an online card.
Delta Dental does not mail you a permanent ID card for adult dental (members age 19 and older). See the instructions above to access and print an online card.
