What to Expect:  2026-27 Out-of-Pocket Costs

The Student Health Benefits Plan uses a tiered provider network.  Students have access to providers in all tiers and your out of pockets costs are based on the tier of the provider.   Your out of pocket expenses will be lower when you receive care from Tier 1 and Tier 2 providers. 

$ Tier 1: Lowest Cost (On-Campus)


$$ Tier 2: In-Network / Select Providers (Off Campus)

  • $20 copay for primary and specialty care office visits
  • $25 copay for urgent care office visits
  • 10% coinsurance for most services
  • $0 coinsurance for covered preventive services
  • $200 deductible
  • $4,000 out-of-pocket maximum

$$$ Tier 3: In-Network / Preferred Providers (Off Campus)

  • 40% coinsurance for most services
  • $0 coinsurance for covered preventive services
  • $600 deductible
  • $7,000 out-of-pocket maximum

  • 40% coinsurance for most services
  • 40% coinsurance for covered preventive services
  • $600 deductible
  • $7,000 out-of-pocket maximum

Where You'll Use It Most - Understanding the Provider Network

$ Tier 1: Lowest Cost (On-Campus)


$$ Tier 2: In-Network / Select Providers (Off Campus)

  • The Student Health Benefit plan uses a PPO network
  • Inside Franklin County, students use the Ohio State Health Plan Network
  • Outside Franklin County, students use the UHC Choice Plus Network which works nationwide
  • No referrals are required


$$$ Tier 3: In-Network / Preferred Providers (Off Campus)

  • The Student Health Benefit plan uses a PPO network
  • Inside Franklin County, UHC Choice Plus Network but not Ohio State Health Plan Network
  • No referrals are required



$$$$ Tier 4: Out-of-Network Providers (Off Campus)

  • All other providers
