Welcome to the 2018-19 SHI Benefits Plan!

Spring/Summer coverage is effective 01/01/2019 to 08/12/2019.

Wanted to waive SHI because you have other active annual health insurance but forgot?  Domestic students who missed the 12/31/18 Select/Waive deadline should complete a Petition to Waive form.

Need a prescription soon?  It may take up to five (5) business days from the date you become actively enrolled before pharmacy systems can verify your SHI coverage. Options include working with the pharmacist to dispense a smaller initial amount and the rest later; asking your prescribing doctor for a short-term sample; or pay in full up front and submit a claim for reimbursement later.

Is there anything I need to do right now?

  • Print your Member ID card.  It is your proof of insurance. You can find instructions for printing your medical and dental id cards here
  • Verify and/or update your “Home/Curr” Address in My Buckeye Link.  Our policy partners use this address for important communications such as requests for additional information and Explanation of Benefits.   
  • Domestic students should complete the secure Coordination of Benefits online HealthSmart form within 30 days to ensure claims process correctly.  International students do not need to complete the form. 

Where can I get more details on the SHI Benefits Plan? 

  • The Member Overview Booklet highlights commonly used coverage in an easy-to-read format. Use it for quick reference.
  • The Summary Brochures define your policy's coverage and exclusions. Read these documents for full details. 

I need help finding a provider.  Your provider choices can help you save money. 

  • Tier 2:  If those are not available, try providers designated as Tier 2 - where some services are subject to a deductible and copays and coinsurance apply. 
  • Tiers 3 and 4:  Seeking providers in higher tiers 3 and 4 will likely result in much higher out-of-pocket costs.

What is a formulary and does my pharmacy benefit use one?  A formulary is a list of prescription medications (generic and brand) covered by the SHI Benefits Plan.  Medications listed on the formulary most often result in lower co-insurance for the plan member.  When you fill a prescription at the Wilce Student Health Center Pharmacy or any HealthSmart RX Network Pharmacy, you pay only the co-insurance and applicable minimum cost.   

  • The SHI Benefits Plan formulary is managed by HealthSmart Rx and updated each year.  The 2019 Formulary is effective January 1, 2019.  For additional information, please see review the 2018-19 SHI Benefits Plan page found here
  • All specialty medications must be filled through Accredo Specialty Pharmacy.  Accredo does not have storefront locations. Medications are shipped directly to you.  Patients can manage their prescriptions via patient portal at accredo.com or by contacting Accredo at 877-222-7336.  Providers can contact Accredo at 866-716-7204 for assistance with prescriptions for their patients. 

Under the age of 19 - understanding your Pediatric Dental benefit. 

  • UHCSR provides primary coverage for members under 19 years of age, and Delta Dental of Ohio provides secondary coverage. Dental providers can seek reimbursement payments from both HealthSmart and Delta Dental, which can in turn reduce your financial responsibility. 
  • How it works if you're under 19:
    • You need to make sure you utilize a dental provider who is part of the Delta Dental Premier and PPO Networks, or at Student Health Services and College of Dentistry Student Clinic. 
    • At your appointment, show them both your UHCSR and Delta Dental Member ID cards. ?Ask the provider to submit claims first to HealthSmart (via the information on the back of your UHCSR card) and next to Delta Dental. 

Away from campus - travel with confidence.

  • You have UnitedHealthcare Global coverage for travel and medical assistance, evacuation and repatriation when you are 100+ miles from home. Note: For International Student members, the Global coverage is not active when you are in your home country.
  • If you travel overseas, you can also submit medical care bills for reimbursement at our Tier 2 coverage level.
