Greetings Student Health Benefits Plan Members!

 

Here Are a Few Tips for Using Your Benefits.

WHERE CAN I FIND A PROVIDER? Visit our Find a Provider page to search for a provider on campus, in Franklin County, or across the state/country.

LOOKING FOR AN ONLINE PROVIDER? You have access to HealthiestYou for 24/7 virtual access to licensed medical and mental health providers at no out of pocket cost to you.  Get started by accessing online at HealthiestYou or by calling 1.855.933.2121. 

o    If you are outside the United States and require medical care, please call UnitedHealthcare Global to arrange services you need to ensure you receive the correct benefits. Please check the back of your Member ID for additional details.   

REMEMBER TO TAKE YOUR HEALTH or DENTAL INSURANCE ID CARD WITH YOU when you get care. If you need an ID card - visit our Member ID page for details on accessing your card, and make sure you have set up your My Account member portal from United Healthcare Student Resources.

DID YOU GET A BILL from a provider OR an EOB (explanation of benefits) from UnitedHealthCare Student Resources and have questions about it? If you receive documents or bills that you don’t understand, please see our Claims Assistance page, call/email our office, or contact UHCSR at t at 1.833.789.9300. 

·         Remember that deductible, co-pay or co-insurance amounts may apply to some or all services – depending on where your go for services. 

·         For Tier 2 providers, many covered services have a $20 co-pay, 10% co-insurance and/or are subject to the $150 individual deductible.

PRESCRIPTION DRUG SAVINGS:  Please see our Prescription Drug Savings article to review options for saving money on your prescription drug needs.

Benefit Reminders

  • SEASONAL FLU SHOT:  Your plan covers 100% of a seasonal flu shot at many provider and pharmacy locations. Seasonal flu shots at non-network provider or pharmacy locations are covered at 60%.
  • DENTAL CLEANING AND EXAM:  Your plan covers two (2) dental cleanings and exams per plan year. Services at Student Health Services or the OSU College of Dentistry Student Clinic are paid at 100% after a $17 co-pay.

ROUTINE MEDICAL SERVICES:  Office visits and diagnostic laboratory and x-ray services used to diagnose or treat a covered illness or injury are covered at 100% when performed by providers at Student Health Services - where deductible, co-pay and co-insurance amounts are waived for most services.  For a more detailed list of available benefits, please review our MEMBER OVERVIEW BOOKLET.
